Improve the default ISUPPORT tokens.

- Replace the deprecated MAXCHANNELS token with CHANLIMIT.
- Remove the deprecated FNC token (other implementations can force
  a nick change without sending this token so informing clients of
  that we might force a nick change is pointless)
